The purchaser of a unit at Como Condos may name a dependable individual on this form to pick up their keys and FOBS on their behalf.
By submitting this form, the buyer authorizes DeSantis Homes to release their keys and FOBs to the designate specified below.

NOTE: On the day of pickup, the designate must bring identification that matches the uploaded file or said keys and FOBs WILL NOT be released.
